Complete the Consultation Request Form
to submit the following information:
- Desired date and time for your private Zoom Consultation with Sherry
- Length of consultation (45 minutes, 60 minutes, or 90 minutes)
- Type of Session you want
- Your date of birth
- Your time of birth (ex: 3:14 am) Note: If you do not know your time of birth, results will be less specific.
- Your place of birth (city, state, country)
- For a Synastry Session (relationship reading), provide the birth date, time and place information for the 2nd person.
- Enter Promo Code for Monthly Special if applies (optional)
